Senior Trip – May 31 to June 2, 2012

As a celebration of community and years of collaborative work, our seniors are given the opportunity to journey to Southern California for our annual three day Senior Trip.  Staff and students will spend an evening at Disneyland/California Adventure Park, play at Newport Beach, visit a zoo, take part in the final team-building activities of their high school careers, and feast at a luau.

This trip is a wonderful time when the entire class comes together to reflect upon their time at Da Vinci, build community, and celebrate prior to their graduation ceremony.  In order to participate in the trip, students must maintain good grades, have no major discipline or attendance issues, and must be on track for graduation.

Our seventh annual Senior Trip to southern California will take place Thursday, May 31, 7:15 am – Saturday, June 2, 2012, 11:30pm.

Our wonderful senior teachers, Scott Steven Bell, Tyler Millsap and Lynn Fowler will be spending their Memorial Day cooking for the big BBQ on Senior Trip. They would like to borrow your ice chests to keep the food frozen until the big day.

Please lend them your ice chest(s), to be delivered to the Da Vinci High School Office just before the Senior Awards Ceremony on Thursday, May 24 between 7:00 – 7:30 pm. (Award Ceremony begins at 7:30 pm.).

Senior trip is just two weeks away! Our trip itinerary with notes is below for your reference.

  • Students must be at school by 7:15am on Thursday, May 31 to check in, with a bag lunch (and a bag breakfast if they haven’t had any).
  • Students will return home late Saturday night/early Sunday morning depending on traffic.
  • All costs are covered, and all meals provided except the ones mentioned above.
  • Students can bring spending money if they wish, but will not need any for food or admission costs.

Trip Notes:

  • Bring bag lunch (& breakfast if they haven’t eaten). No food stops, just bathroom stops on way.
  • All other meals and costs included. Students should not need money for basics.
  • 7:15am: Please arrive on time.
  • Students check in with Mr Bell or Ms Fowler
  • Bus to leave promptly at 8 am.

Medications- Only medications previously received and approved by our school nurse allowed. Students are NOT allowed to carry medication (exception albuterol inhalers).

Clothing-

  • Chilly at night, need pants & sweatshirt
  • Plain clothing for Disneyland, no logos of any kind allowed, no flip-flops, open toed shoes.
  • Bring bathing suit & towel, whether you think you want to wear it or not. Sunscreen provided.

Bring carefully labeled beach items to share: beach chairs & umbrellas, beach toys (Frisbees, nerf footballs, beach balls, bocci ball set etc), blankets, beach towels.

Medications for Senior Trip, with signed paperwork already on file with me in the Da Vinci Office, are due tomorrow, Thursday, May 3 or Friday, May 4, during the times listed below. If you cannot come at either of these times, please email Marguerite Fleming (mfleming@djusd.net) to find a time when medications can be checked in while I’m here.

A parent or guardian MUST deliver medication to school, as students may not have any medication in their possession on school property.

Senior Trip Medication Forms Overdue:Just a reminder that Senior Trip Medication Forms are overdue in the office. Students taking regular daily medication should take their medication during the trip. Student medications require mandatory paperwork to be completed by parent and physician and should be delivered or mailed to the student’s physician right away.

These forms are due back to Da Vinci by THIS Friday, April 6. Physician’s offices can fax the completed forms directly to the Da Vinci Office at 530-759-2178. Medication taken daily should be taken on the trip. Please send only necessary medications on the trip, as it is a major undertaking to organize meds for over 100 students. If your student has medication or a medical plan at school, that paperwork and medications will be taken on the trip. Medical information is confidential with staff. Medications will be due in the office May 3-4.

We strongly urge you to submit a form for ibuprofen or Tylenol if your student takes these medications. It is requested very frequently by our seniors especially for sunburn (despite our efforts to continually urge them to apply sunscreen). If your student has had motion sickness issues in the past, please complete the form giving staff permission to administer that medication.

The actual field trip form (double sided) is past due! Please ask your student for this form requiring your signature (even if your student is already 18).

BLUE FIELD TRIP FORM
The blue field trip form from the senior trip packet was due last Friday. It is the only form now due. These forms will give us a more accurate count for purchasing Disneyland tickets. While Disneyland has reserved enough tickets for the entire class, we must pay for them now, and they are non-refundable. There is no guarantee that we can purchase more after the deadline. We are behind on our fundraising, and do not want to make unnecessary purchases, nor do we want to leave anyone behind for lack of a ticket.

Student Medications

Please take or send these forms to your student’s physician immediately. The physician’s office can fax the forms back to us at 530-759-2178. Medical information is confidential with school staff.

A form MUST be completed for every single over the counter and prescription medication that a student will take during the senior trip.

Most members of last year’s senior class requested at least one of these medications during the trip, especially for sunburn (even though staff sprayed sunscreen on them frequently). Staff CANNOT provide any medication unless the proper paperwork is on file.

Each year we get the most questions about the medication forms. Please see below for detailed information.

MEDICATION FORMS

A district medication form MUST be on file for any medication, prescription or over the counter, that a student needs for the senior trip.

  • All the blanks must be filled in legibly.
  • The parent AND physician MUST sign the form.
  • It does not matter if the student is 18, or if it is an over the counter medication, we must have the signed form to administer a medication.
  • Students will NOT be allowed to carry their own medication according to district rules.
  • Emergency asthma inhalers (albuterol) are the exception.
  • Students should bring only necessary, daily medications on the trip, as it is a complicated process to carry/track all medications.
  • Forms should be mailed or brought to your student’s physician’s office, and can be faxed to us at 530-759-2178.

GOLD FORM FOR IBUPROFEN, TYLENOL, OR MOTION SICKNESS MEDICATION (DRAMAMINE) – Due April 6
Please strongly consider submitting this form if your student takes ibuprofen or Tylenol for minor aches and pains. Most students request these medications, especially for sunburn, even though staff continually remind students of sunscreen availability.

Write “as needed for minor aches and pains” in the Diagnosis space.

If your student has any history of motion sickness, please submit the form for the motion sickness medication. There is nothing worse than being sick all the way to/from this amazing trip.

PINK MEDICATION FORM – Due April 6
Please use this for all other medications your student takes regularly.

The first payment and pledge coupon are due 9/30/11.

When budgeting for the amount of your donation, please keep in mind that:
Some families can afford to donate more than others, and all costs must be covered
Satisfying the $35,000 with this activity negates the necessity for smaller, multiple fundraisers and the energy necessary to make them happen. The Senior Trip could be paid in full by the end of 2011 and you can focus on other memory-making activities with your 2012 senior.
